Our office is in the Visitors Center of Medical City, which is in the Atrium of Building A. We are open Monday - Friday from 9:00 a.m. - 5:00 p.m. Come by and complete an application or call (972) 566-6816 and we will send membership information.

Cost of Membership
- $15 per year
- $25 for a two-year membership or two one-year memberships for individuals at the same address
